Twilight’s his Guardian and Spike’s her Ward.  
Simple, factual, and yet so vague it doesn’t really define their actual relationship.  
But, seriously, she doesn’t treat him like Shining (so not like her actual brother), but also isn’t as protective as you’d expect of a mother (even AJ’s more protective of AB)  
She was told to take care of him, and she did, and in the later years he apparently enjoys helping her, so he hasn’t left yet - it’s a unique not-blood-related relationship, really.  
Trying to equate it to ordinary family would mean missing out on important parts of their relationship.
