Definitely twin siblings. Unless they’re the same pony buy they got a sex change and the same cutie-mark reassignment surgery Prince Blueblood got (explaination: Prince Blueblood has the wrong cutie mark in the Equestria Games episode)

Personally, not really a fan of the name Old Gem, or even just Gem. If her brother’s name is Karat, perhaps her name should be Clarity (or Klarity, if their parents were really on the common-initial thing). Karat and Clarity are two ways to describe the purity of a gemstone.
