Apparently (as best as I can tell after the whole of ten seconds on Google, at any rate) and yes, respectively.
An actual hermaphrodite has full sets of fully functional male and female genitalia.
Many if not most plants and some animals, like earthworms and snails, are hermaphrodites, but as I understand it, true functional hermaphroditism is practically impossible in, say, mammals, due to the genitals for either gender being homologous.
Transgender refers to someone who identifies as the gender opposite to their biological sex. (Yes, that’s a bit of an oversimplification, but I’m not an expert, nor is this really the place for long essays on the varied nuances of gender identity anyway.)

Actually, technically, this is futa, no matter what the site rules say. Much as I love the site, Derpibooru isn’t in the business of redefining words :P
“Futa” is short for the Japanese futanari, (“[of] two kinds,” or somesuch) which apparently has been used as an umbrella term for any and all that could possibly be considered even remotely androgynous; from hermaphrodites to intersex to transgender to transvestism to tomboys.
Which is exactly why it’s an utterly, infuriatingly, useless tag that should be done away with entirely and replaced with the unambiguous “hermaphrodite” for clarity.

Actually, surprisingly enough, this isn’t futa. According to TSP for tagging purposes on this site, if they only have male genitalia, they’re considered to be male or transgender. To be futa, they need to have both a penis and vagina.
And all I see below Rainbow’s and Fluttershy’s anuses are delicious taints and balls.
