Shouldn’t this be safe? There might be no evidence of clothing, but there’s also no evidence of nudity. Nothing’s showing. Or is this sufficient cleavage/decolletage for “suggestive”?

I’d say yes because that kind of shading detail or fabric creasing is present nowhere else on the image other than the stomach. That outfit is skintight, and they put that detail there for a reason.
