Venom Snake, aka the “Big Boss” you play as in MGSV:TPP, is secretly the medic seen in MGSV:GZ, who had undergone major surgery and hypnotherapy to make him look and believe as if he were the real Big Boss.

Thus, Venom Twilight (or any of the others as Venom!Boss, unless it’s otherwise noted, implied, or Pinkie with an ice cream cone on her head) is automatically Nurse Redheart.
