Then that would support Twilight as the Medic more as the Medic is typically found healing his allies near the front lines where he is needed most. The Medic is by his nature a thought class, and though it’s not ideal for him to be fighting, he can defend himself in a pinch. While Twilight is certainly capable of fighting, she has many utility and support abilities at her disposal such as force fields, transformation, enchantment, and more.
 
As for Fluttershy, she’s never done anything relating to combat support. Her personality is one to avoid danger, which would place her closer to the Sniper and the Engineer, who typically find good sniper perches and sentry nest locations respectively. The Spy might also work, because while he typically goes towards the danger, he also does so in disguise or cloak, fitting her shy nature.

I’m not sure I understand why you think Twilight wouldn’t be a healer. She has shown some competence in medicine after all, being called for help by Nurse Redheart on one occasion and being approached by Applejack for Apple Bloom’s cutie-pox. Incidents like this sort of suggest Twilight knows enough about medicine to be useful.
 
 
As for Fluttershy, she’s not really any more of a doctor than Twilight is, just an animal caretaker, and most of her treatments are more along the lines of natural remedies than actual medicine. About the only thing that can be said for her is that she knows more about animals than Twilight, which makes sense given her vocation.

Actually yes, it shows que the medical class not depend being necessarily a unicorn and the sniper not depend being necessarily a Pegasus . twilight would not necessarily be the best choice for medical class, and even in tf2 the doctor with his offensive arsenal the his triumph is heal, and even his strong offensive arsenal so serves for escape and not to fight, because the doctor depends of a partner and is a class that looks good to fluttershy by her sweet and helpful way for caring wellbeing that much takes the concept of a doctor helping the neighbor.
